Abstract

Objective: Posterior reversible encephalopathy syndrome (PRES) also known as hypertensive encephalopathy is a clinico radiological entity. Eclampsia is one of the important causes. Most of the patients have severe hypertension, some have only mild elevated or even normal blood pressure. It is a neuro toxic state that occurs secondary to the inability of posterior circulation of brain to auto regulate in response to acute changes in blood pressure (malignant hypertension) and is completely reversible. This is an obstetric emergency frequently occurring in postpartum period. It presents clinically as headache, visual changes, altered mental status and seizures in conjunction with radiologic findings of posterior cerebral white matter edema. Magnetic resonance imaging (MRI) is the gold standard test. The condition resolves within 1 – 2 weeks when the appropriate cause is treated. We report a case of 19yr old female, with unremarkable antenatal record, who developed convulsions in the immediate postpartum period following emergency cesarean delivery.
