Purpose:Congenital diaphragmatic eventration is a rare diaphragmatic anomaly. Left side most commonly involved. Congenital diaphragmatic eventration may result from congenital phrenic nerve injury or thin abnormal diaphragm at birth. The newborn may present with dyspnea, cyanosis and tachypnoea. Here we present a 1 day old term male child born following an uncomplicated pregnancy and delivery and who presented with shortness of breath, laboured respiration and chest retractions. Chest X ray and CT chest revealed an elevated right hemidiaphragm. The neonate was initially managed on bubble CPAP (continuous positive airway pressure), as respiratory distress was not subsiding, supported with mechanical ventilation. Weaning of mechanical ventilation was difficult. Diaphragmatic plication was done and the baby was discharged on the day18th of life.
